Open Enrollment OUT to another District (e.g., Resident student, but attends Jackson-Milton, Weathersfield etc.)
Please complete the Registration Packet (can be picked up at Registration Office or downloaded from our website), include birth certificate and proof of residency.  No appointment is necessary. Drop off information and your student will be registered in Austintown but shown as attending another district.

(Valid proof of residency must be attached to this form to be valid)

Consent for Release of Records

(Must be signed by parent/guardian and sent to the previous school in order for records to be sent to us.)

Emergency Medical Form

(Complete and return during the time of your registration)

Official School Withdrawal

(Please complete, sign and return to school if withdrawing from ALSD.)
